The Innovative Smiles Clinic Membership Plan is an annual membership plan that allows individuals and families who do not have dental insurance to receive quality dental care at a reduced cost.

Our in-office membership plan offers straightforward savings without many of the restrictions associated with traditional dental insurance.
If you use CareCredit or another third-party financing option to pay for services, the discount will be 20% instead of 25%.

You will not receive a membership card. Your plan’s effective date will be kept on file with our office.
This program is an in-office discount plan, not a dental insurance plan.
This program cannot be used:
This plan is valid only at the office of Innovative Smiles and cannot be used at another office. Children listed on the plan must live in the same household or be full-time students under 18 years old.
This plan is non-refundable. No refunds or premiums will be issued if a participant decides not to use the dental plan.
The 12-month membership is due in full upon joining and becomes effective on the date payment is received. Payment for treatment is due when services are rendered.
It is the member’s responsibility to arrange appropriate appointments within the 12-month membership period. Unused appointments or services do not qualify for a refund.
Renewal payment is due at the beginning of the same month in which the member originally joined. Membership rates may be reviewed and adjusted annually with notification.
The membership plan excludes panoramic and cone beam X-rays. Membership expires one year after joining the program.
